Thank you for considering Healthy Expectations for physical therapy care for your patients. We specialize in the treatment of male and female pelvic floor dysfunction, women’s health physical therapy, pediatric pelvic floor dysfunction, and a broad spectrum of orthopedic conditions.

The referral needs only to state the patient's name, the date, the diagnosis, and your signature. You may indicate "evaluate and treat", or feel free to give more specific instructions, as desired. Healthy Expectations prescription pads are also available. Our prescription pads include a list of diagnoses that we commonly treat, our contact information, and a convenient map to our location. Please call us to request prescription pads for your office.

Next, please fax us a copy of your referral to (260) 483-9931 with the patient’s phone number so that we may contact the patient to set up an initial evaluation. The patient may also call us at his/her convenience to set up an appointment. Faxing a copy of the referral allows us to keep you up-to-date regarding your patient. If the patient decides not to pursue physical therapy, we will send you a note stating this and the reason why.

Once your patient is seen in physical therapy, we will send a copy of our initial plan of care. Your signature is required on this plan of care. Please sign it and return via fax or mail. To keep you updated on your patient’s progress, we will also send regular progress notes. These, too, require your signature, certifying the plan of care. When your patient has completed therapy, you will get a discharge summary informing you of the current status of your patient. This summary is for your information, and does not need to be signed.

Patients who commonly benefit from individualized physical therapy treatment include:

Pelvic Pain: Patients with chronic abdominal, pelvic, vulvar or penile/testicular pain, pain with intercourse, pain with pelvic exam, pelvic floor tension, rectal pain, coccydynia, vaginismus, vulvodynia, or interstitial cystitis.

Pelvic Floor Muscle Weakness: Patients with stress incontinence, urge incontinence, urinary urgency and/or frequency, fecal or anal incontinence, prolapse of the bladder, rectum, or uterus, or post-surgical urinary or fecal incontinence.

Pregnancy and Post-partum Issues: Patients with back, hip, or pelvic pain, sciatica, rib pain, thoracic outlet or carpal tunnel syndrome. Prenatal or post-partum patients with difficulty performing normal daily activities like walking, bending, lifting, pushing and pulling, impaired ability to perform child care and work activities, impaired ability to sleep due to pain, patients with persistent vaginal pain after episiotomy or perineal laceration, postnatal stress urinary incontinence and pelvic organ prolapse, post-cesarean abdominal pain and weakness.

Post-surgical: Male or female patients who have undergone gynecologic, urogenital, rectal, cancer treatment, abdominal, or joint surgery who have pain or tenderness that lingers or is more intense than expected, patients who have difficulty returning to normal daily activities, those with complaints of restricted motion or a sensation of 'pulling' in the area of the incision, abnormal scarring or adhesions, excessive swelling, and post-surgical muscle weakness.

Orthopedic Conditions: Patients with orthopedic conditions such as sprains, strains, low back pain, neck pain, sciatica, whiplash, tendinitis, bursitis, arthritis, headaches, shoulder and arm pain, hip, knee, ankle, and foot pain; whether these conditions are due to acute injury, chronic overuse, work-related, sports-related, due to an automobile accident, or post-surgical.

Other Conditions: We also provide treatment for osteoporosis, and various colo-rectal conditions including constipation, obstructed defecation, and rectal pain.
